Objectives – To propose a neuropsychological study of the various aspects of self-consciousness (SC) in Alzheimer’s disease. Methods – Forty-five patients with probable mild or moderate AD were included in the study. Severity of their dementia was assessed by the Mini Mental State (MMS). Fourteen questions were prepared to evaluate SC. Results – No (...) significant correlations were found between SC score and educational level, age, and duration of disease. A significant correlation was found between SC score and the severity of dementia, whereas frontal disturbances were just short of the significance threshold. The various aspects of SC were not impaired to the same degree. The most disturbed ones were awareness of cognitive deficiencies, moral judgements and prospective memory. The least disturbed aspects were awareness of identity and of mental representation of the body. Items relating to anosognosia and moral judgements were significantly correlated with the MMS score, whereas affective state, body representation disorders, prospective memory, and capacities for introspection were not related to the severity of the dementia. Consciousness of identity was sound, regardless of MMS score. Conclusions – AD clearly induces an heterogeneous impairment of SC. SC requires a convergence of many neural networks. In AD, neuronal alterations involve many cortical areas and information sent to the associative frontal cortex from memory, language and visuospatial areas is lacking or disturbed. Thus, the sequential order of successive stimuli cannot be maintained by the heteromodal associative cortex (dorsal convexity of the prefrontal cortex), and the supramodal associative cortex (located rostrally in the frontal lobes) is unable to provide reliable monitoring and assessment of simultaneous neural cognitive networks carrying insufficient and inadequate input. The core deficiency in AD patients might be impaired SC equated with the disability to maintain sequential and simultaneous ‘‘attention to life’’. The Self-Consciousness Questionnaire, a clinical scale providing multidimensional measurement, indicates that different aspects of consciousness are not correlated with overall cognitive deficiency as determined by the MMSE. The conclusions of a working party set up by the British Association for the Advancement of Science are embodied in a small book reviewed on pate 108 of this issue, but we feel that more discussion than can be set out in the space allocated to book reviews is justified. Also, this journal in its first number has already devoted (...) a group of papers to the issue of abortion, so the conclusions of the working party and our own comments overlap and we hope illuminate further the problems enunciated. The health benefits of male circumcision are wide rangingIn this issue, John Hutson has reiterated the conventional Western medical view that “the surgical argument for circumcision of all neonatal males at present is very weak” and he criticises many of the circumcisions performed in later childhood, without anaesthesia, as “physically cruel and potentially dangerous” [see page 238].1 He is also of the opinion that “the diseases which circumcision is able to prevent are uncommon or even rare”. But therein he errs, (...) and greatly errs.He cites only two publications dealing with the protective effect of male circumcision against HIV infection, and makes no mention of the important recent meta-analysis of Weiss, Quigley, and Hayes2 which shows conclusively from a large number of studies that …. Roger North's The Musicall Grammarian 1728 is a treatise on musical eloquence in all its branches. Of its five parts, I and II, on the orthoepy, orthography and syntax of music, constitute a grammar; III and IV, on the arts of invention and communication, form a rhetoric; and V, on etymology, consists of a history. Professor Sir Roger Penrose's work, spanning fifty years of science, with over five thousand pages and more than three hundred papers, has been collected together for the first time and arranged chronologically over six volumes, each with an introduction from the author. Where relevant, individual papers also come with specific introductions or notes. Many important realizations concerning twistor theory occurred during the short period of this third volume, providing a new perspective on the way that mathematical features of (...) the complex geometry of twistor theory relate to actual physical fields. Following on from the nonlinear graviton construction, a twistor construction was found for (anti-)self-dual electromagnetism allowing the general (anti-)self-dual Yang-Mills field to be obtained. It became clear that some features of twistor contour integrals could be understood in terms of holomorphic sheaf cohomology. During this period, the Oxford research group founded the informal publication, Twistor Newsletter. This volume also contains the influential Weyl curvature hypothesis and new forms of Penrose tiles. In the United States, a longstanding legal rule exists against patenting natural phenomena. The Supreme Court recently had an opportunity to help define the boundaries and clarify the implications of this “natural phenomenon doctrine” in Laboratory Corporation of America v. Metabolite Labs., dismissed as improvidently granted. This article argues that the natural phenomenon doctrine renders both the patent claim at issue in LabCorp, and the patents that directly or indirectly claim biological correlations between genotypes and medical phenotypes, invalid or unenforceable (...) under U.S. patent law. Benedict de Spinoza was at once the father of the Enlightenment and the last sad guardian of the medieval world. In his brilliant synthesis of geometrical method, religious sentiment, and secular science, he attempted to reconcile the conflicting moral and intellectual demands of his epoch, and to present a vision of humanity as simultaneously bound by necessity and eternally free.
